The post is ideal for an enthusiastic individual who would like join a friendly, vibrant and forward thinking team. It would suit someone interested in integrated working who is committed to developing and expanding their skills to achieve the best quality care and outcomes for patients in their own homes.Through collaboration with a range of health and social care professionals, the post holder will work collaboratively with patients and others to promote self-management, early intervention and, where needed, improve support for the older person and those with frailty, along with involvement in discharge planningThe role includes home visiting; providing interventions to patients within their own home, involving lone working without direct supervision. You will liaise with a wide range of Health & Social Care professionals to identify solutions for patients in a multidisciplinary network. Delivering training to relevant staff groups will also be included, and participation in audit processes .In return we offer varied and interesting work in an environment of excellence and innovation.
