Are you experienced at rostering and supervising staff? Have you had experience working in health or social care?If you have answered yes to these questions then we have this highly rewarding post available and would be thrilled to welcome you to the Neath Port Talbot Community Resource Team. We are an integrated service across health and social care but we also benefit from good relations with our primary care colleagues and the third sector.You will work as part of a team of Flow Facilitators whose main responsiblities include:- Scheduling Reablement calls for people returning home from hospital and those who may be at risk of admission to hospital or residential care- Liasing with professionals across a community and hospital setting, families and patients to ensure people transition safely between services- Supporting and supervising support worker staffWe are able to provide a strong and supportive working environment that seeks to deliver a highly effective service in line with Swansea Bay University Healthboard’s values – caring for each other, working together and always improving.This post is offered on a permanent basis for 22 hours a week although we may have opportunities to offer additional hours on a temporary basis. The post does involve working as part of a 7 day working pattern and as part of an on call rota.Being able to speak Welsh is desirable for this post.This advert closes on Tuesday 22 Dec 2020
