We are seeking motivated and experienced Community Home Support Service Coordinators to support and enable people to remain independent in the community during times of heightened need. The service will be provided to people who are not eligible for social care support but whose needs are likely to escalate without any intervention. This role will include occasional evening and weekend work.

This is a non-regulated service to support people discharged from hospital and to prevent unnecessary hospital admissions and to provide:

Non-personal care
Low level practical support
Information & signposting to ensure people can live independently in their own communities.
