Pembrokeshire County Council has a new and exciting opportunity for those wishing to work in Social Care. Our Reablement Service is looking to recruit a Care co-ordinator.
The post holder will work as part of a team to deliver person centred care to customers within the community and will take on a care co-ordination role within the service working alongside Team Leaders and the Registered Manager, ensuring quality care plans are delivered and rotas are in place to ensure delivery of care is maintained. Manage and maintain the electronic scheduling system ensuring customer visits are scheduled, carers are assigned appropriately and timings are optimised ensuring both value for money and delivery of high level support.
You will need to hold a Level 3 qualification in a relevant discipline, for example business administration and have experience of working within a business environment.
