At Case Management Cymru we provide highly professional, bespoke case management to adults and children with a range of life-changing conditions/injuries.

The ability to drive with use of a car is essential.
We are looking for a friendly, outgoing and reliable individual to support a young man with a severe Traumatic Brain Injury in his own home and within the community. Having excellent communication skills, you will work as part of a multidisciplinary team, and provide support with the neuro-rehabilitation programme whilst maintaining focus on a goal centred approach.
Our client has a very keen interest in diet and fitness so an interest in these activities will be key. There is a plan for him to return to education in September and he may need support with the additional demands this could bring. He will require support to be able to spend more time with his young child and as a Rehabilitation Coach, you will need to have enthusiasm and be able to engage with all the activities and interests that our client has with an emphasis on maximising his independence.
Whilst experience in this area of work is preferred, it is not essential as a full induction and training will be provided.
This role is subject to an Enhanced level DBS check.
Weekday (7am to 11pm)
Weekends/Bank Holidays (7am to 11pm)
Core hours: 9am to 6pm (Weekdays and Saturday)
