Principal Accountabilities / Key Tasks:
Take the lead in day-to-day operational management of the staff identified by the manager.
To visit service users prior to commencement of service, or within 3 days of service commencement to discuss the care plan, terms and condition of service, complete the service delivery plan and risk assessments.
To carry out review visits to service users to ensure that relevant paperwork in the service user’s file is up to date.
Assist the Home Care Manager in fulfilling the obligations of the agency’s Quality Monitoring Process.
Arranging home care service provision to meet the needs of service users and informal carers who have been assessed. To include the processing of care plans by commissioners to produce a service delivery plans which is effective and of a high quality.
Ensure the effective allocation of work within the Team, ensuring that cases are prioritised are adhered to.
Responsible for supervising staff who may be dealing with complex clients.
Managing risk for staff and families.
Planning and producing the weekly staff rotas and ensuring that home care staff are aware of them. The post holder will need to match the appropriate staff, in terms of location and qualifications, with individual service users based on the assessed needs as a central aspect of this process, operated through the information technology system.
Processing schedules of staff. This will include liaison with Payroll to ensure correct payment to staff.
To supervise staff effectively on a 3 monthly basis and review performance on an annual basis in accordance with statutory requirements.
Identify staff training needs.
Monitor sickness absence.
Contribute to the recruitment and selection of new members of the team.
Undertake induction of new members of staff.
Effectively respond to telephone inquiries and other inquiries about the provision of home care services. This will include receiving and responding to complaints and refer any staff compliments to the Home Care Manager.
Assist Home Care Manager in gathering information / evidence in respect of complaints and investigations.
Hold monthly team meetings with staff.
Identify any training and development needs and feed into workforce development plans.
Attend monthly team meetings as arranged by Home Care Manager/Business Manager.
To attend CRT weekly meetings.
To participate in the departments Quality Assurance process which includes undertaking Quality Monitoring visits in other patches.
The post holder is expected to be aware of the requirements of the Health and Safety Acts and take responsibility for general health and safety and to ensure that staff are undertaking correct manual handling procedures.
In an emergency situation undertake hands-on duties until suitable carers are found.
To be part of the Out of Hours Team on a rota basis to offer support to service users / carers, care workers and health care proffessionals.
To provide Out of Hours cover if other colleagues are on sick leave or annual leave.
To work within the values and principles of the Home Care service as specified in the Care Council for Wales Code of Conduct for Carers.
To undertake training and other staff development opportunities as specified by Home Care Manager.
