Purpose of the role:
Livability has an exciting new opportunity in Wrexham supporting adults with learning disabilities to live independently in their own homes.
As a Service Delivery Leader you will ensure that regulatory requirements are met across the service taking full responsibility for the management of the service providing the highest quality of care and support. Working with the delivery team and Quality Improvement Team to ensure the services are rated good or outstanding or equivalent depending on the regulator Team. This position requires you to be able to register with Social Care Wales.
You will be leading a team of Enabling Support Workers to work with people using Positive Behaviour Support and Active Support – supporting to maintain and promote independence, to safely access their community and engage in meaningful activities inside and outside of their home.
You will be supporting the team to enable people to carry out everyday activities, promote structure and routine to increase their independence, and to maintain their home within personal living spaces and communal areas.
The role will also involve supporting individuals with personal and health care in accordance with individually assessed needs.
