Team Leader – Supported Homes Isle of Anglesey County Council

To deputise, in the absence of the Supported Living Manager, in the day to day operation of the 3 projects and the move on project at Llawr y Dref.

To manage on a day to day basis the Supported Housing projects including the allocation of workload, supervision and training of staff.

To take responsibility for the duration of the shift worked and delegate tasks to support workers as agreed with the Supported Living Manager.

To take responsibility for the overall care and support of tenants residing within the service to ensure compliance with care and support plans as directed by the Supported Living Manager.

To work with individual tenants to achieve their desired outcomes as identified in their care and support plans, complete personal plans with each individual and monitor and review every 3 months or earlier if there are any changes in their care and support needs.

Support tenants to achieve their goals and aspirations as identified in their supporting people housing management plan and monitor progress on a monthly basis.

Ensure that risk assessments are in place and monitored as required and ensure that staff adhere to these.

To take responsibility for other allocated tasks as deemed appropriate by the Supported Living manager.

To provide direct and practical help, care and support in the implementation of agreed packages of care and support to individuals which will assist the tenant in their daily lives.

To provide holistic care as necessary around the individual needs of the person, this will include personal care tasks.

To promote a culture of dignity, equality and diversity and ensure all staff working in the projects treat all tenants with dignity, respect and assist them to undertake a range of
everyday living skills, to access a range of opportunities and experiences either in their own home or the wider community appropriate to their level of functioning, in order to help the individual achieve a dignified and meaningful lifestyle which maximises their independence.

To work in and to promote the active support/reablement ethos, to develop skills to enable tenants to manage their daily lives, including a range of problem solving skills.

Support tenants to manage their medication, including ordering, recording and administering where appropriate. Complete monthly medication sheets.

Support tenants and encourage the following:
• Opportunities to work towards supported employment or volunteering.
• Activities which promote well-being through healthy living and exercise.
• Lifelong learning and the development of skills for independent living and employment.
• Appropriate opportunities that could include arts, cultural and leisure activities.

To work towards goal directed support planning with each tenant

Monitor progress of each indivdual against their goals, and participate in progress reviews.

Support and actively listen to tenants to express their views, make choices and decisions about their lives and assist to facilitate tenants meetings.

To maintain accurate record systems, prepare reports and contribute to multi-disciplinary meetings, under the supervision of the Supported Living Manager.

Communicate effectively both in writing and verbally, with tenants and colleagues, maintaining confidentiality at all times.

To undertake flexible working arrangement in accordance with tenants and the Service requirements, including weekends, evening, nights, sleep ins.

Undertake duties in other Community projects as directed by the Supported Living Manager.

Ensure that all housing management weekly tests are undertaken which include:
• Fire drills and tests as per fire risk assessment
• Legionnaires tests as per risk assessment
• Daily fridge and freezer tempreture checks.
