An opportunity has arisen for a Wellbeing support worker to join Ty Nant Du, Day Opportunities on a full-time basis.
We are looking for a highly motivated and enthusiastic person who are looking for the next step in their career. The role is mainly to support with transitioning of children into adult services, completing Person Centred Plans and contributing to their assessment of need.
The successful candidates will be keen to work in a way which focuses on 'what matters' to the individuals that you work for and is able to form and develop relationships with individuals and carers to ensure that they are supported to achieve outcomes that matter to them. You will be initially based at Ty Nant Ddu Resource Centre in Pontypool, however, could be relocated in the Borough in the future.
