Location of post: 1 post in Upper Valleys Cluster and 1 post in Afan Network Cluster
We are seeking two enthusiastic Carers Development Officers to join our Prevention and Early Intervention Team. The successful candidates will be based in either Upper Valley or Afan Cluster Teams.
You will play a pivotal role in identifying unpaid carers in the community. Work in partnership with unpaid carers to promote their rights and well-being. Provide timely intervention by ensuring access to relevant information and support to prevent escalation of need and helping unpaid carers sustain their valuable role in their local community.
The main duties of the post;
Work in partnership with health and social care professionals to identify unpaid carers.
Advocate and promote unpaid carers rights, ensure that their voice is heard.
Act as a resource to provide information and advice guidance to support unpaid carers in the community.
Develop links and partnerships with local community, social care and health projects across the borough to raise the profile of unpaid carers and increase opportunities for support.
To undertake proportionate carers assessments in line with the Social Services and Well-being (Wales) Act 2014 and promote creative solutions s to meet their identified outcomes.
You will have experience of;
Supporting people in a health or social care setting.
Experience and understanding of the issues that affect unpaid carers.
Ability to work in partnership with other organisations and teams to support people to achieve their outcomes.
Working knowledge of assessment and support planning.
Ideally you will have;
Relevant Health or Social Care qualifications at NVQ Level 2 to 3. Or demonstrable relevant experience and a willingness to work towards this qualification.
Good IT knowledge and skills.
