Disability Can Do are seeking a compassionate and motivated Hospital-to-Home Community Connector to deliver an exciting new pilot project supporting disabled adults and older people across Caerphilly County Borough.

Are you compassionate, practical and committed to helping vulnerable adults remain safe and independent at home?

 We are looking for a Hospital-to-Home Community Connector to provide short-term, person-centred support to individuals following discharge from hospital. Working for up to four weeks, you will help people settle back into their homes, reduce risks, rebuild confidence and connect with longer-term support where needed.

 About the role

You will:

• ·Visit individuals in their own homes following hospital discharge
• Provide practical support, including shopping and prescription collection
• Complete wellbeing and home safety checks
• Identify and respond to risks such as falls hazards, self-neglect and isolation
• Offer reassurance, emotional support and regular contact
• ·Promote independence and wellbeing
• Identify safeguarding concerns and escalate appropriately
• Maintain accurate case records
• Work with health, social care and voluntary sector partners
• Support referrals and connections to longer-term services

 About you

You will have:

• Experience supporting vulnerable adults, older people or carers
• An understanding of frailty, isolation and hospital discharge
• Excellent communication and interpersonal skills
• The ability to assess risk and work independently
• Knowledge of safeguarding principles
• A flexible, responsive and solution-focused approach
• A full UK driving licence and access to a vehicle
• Experience in community, health or social care, knowledge of local Caerphilly services, or a relevant health and social care qualification would be desirable.

This role involves lone working and visiting people in their own homes. An enhanced DBS check will be required.
