We are looking for a highly organised, enthusiastic and motivated person to join our new Integrated Reablement Service to support service delivery.
The Integrated Reablement Service (IR) in Torfaen, part of the Adults & Communities Directorate, aims to support residents in leading healthy, fulfilling, and independent lives within their local community or within Ty Glas y Dorlan.
The IR service supports vulnerable individuals with “assessed eligible” needs for social care provision, to be enabled to be as independent as possible in Torfaen to remain safely in their own homes.This service encompasses community homebased, reablement, respite and adult social care functions. The IR service will focus on preventing unnecessary hospital admissions, stepping down from hospital either to their own home or where this is not possible into our short stay unit (Ty Glas y Dorlan) as a step towards home.
The Enablement Team Lead will work in a person-centred outcomes focused manner, being able to model ‘what matters’ conversations and assessment, work with other social care practitioners to embed practice that is strengths based, focused on promoting independence, skills building and positive risk taking. You will direct and guide support workers and reablement practitioners through difficult and challenging cases and support them to deliver a safe and effective services to people living in the community.
The Enablement Lead has a key role in carrying out functions linked directly with Care Inspectorate Wales (CIW) regulations. You must have a well-developed knowledge and understanding around all the relevant legislation relating to Domiciliary Care. The person appointed must demonstrate experience working within the field of social care and be able to display excellent risk management skills. You will direct, supervise, quality assure and safeguard the service which operates over a 24-hour period, 365 days a year.
The successful candidate will be based at Ty Glas Y Dorlan . The post is full time, 37 hours per week, covering office hours between 8:30 am – 5:00 pm Monday to Thursday and 4:30 pm on a Friday. There is also a need to provide management cover and participate in a standby system to cover the out of hours personal care service (Additional Payment will be made). You will therefore be on-call during out of office hours, on a rota basis which covers 7 days a week, 52 weeks a year from 7:am – 8:30 am and 5:00 pm (4:30 pm on a Friday) 11:00 pm.
