This is an exciting time to join our developing Older People's Mental Health Team in Bridgend. We are seeking to appoint a creative, enthusiastic, ambitious full time Social Worker on a permanent basis. The team consists of Social Workers, Senior Social Work Practitioner and Team Manager. Within this role you will work in a multi-disciplinary environment to support the needs of adults over 65 affected by long term complex and adverse mental health conditions including Dementia. The team provide assessment of needs, risk formulation and collaborative Care and Treatment Planning under the Mental Health (Wales) Measure 2010 to those accessing secondary mental health services. The role is predominantly community based with the team also covering inpatient psychiatric wards in Angelton Clinic, Glanrhyd hospital.
The individuals we support are living with complex mental health challenges and may also have physical health and emotional wellbeing needs. You will be required to provide specialist support as part of a multi-disciplinary team to maximise individual's wellbeing and effectively manage identified risks associated with their mental health. The team works with older adults to provide holistic and strengths-based support to allow people to live their lives as they wish, ensuring voice, choice, independence and control. The successful applicant will work alongside Community Mental Health Nurses, Psychiatrists, Specialist Occupational Therapists, Psychologists and Physiotherapists. We also work closely with primary healthcare professionals including G.P. surgeries and the third sector partner agencies to support the needs of individuals accessing support along with their carers and families. Using a strength-based outcome focused model of Social Work, the team supports adults, and their carers to achieve positive outcomes within their lives and encourage wider inclusion in local communities. Maximising an individual's emotional wellbeing is seen as key in effective social work practice. We are keen to continue to develop innovative approaches to social work practice and improve support available to those living with complex mental health and dementia in the local area. We work closely with other agencies and partners to prevent unnecessary admissions to hospital and care homes, as part of our role to maximise independence, choice and providing support in the least restrictive way. The continued development of this dynamic service model will require work on widening partnership opportunities with statutory partners, the third sector and with individuals from the community. We are particularly looking for those who are interested in community work and therapeutic social work practice. We are keen to develop resilient community connections, groups and services in Bridgend. The position will be office based in Bridgend with the team covering the whole of the Bridgend borough.
