Mae angen Ymarferwyr Gwasanaethau Cymdeithasol i ymuno â'r Tîm Gwaith Cymdeithasol Cymunedol Gogledd newydd ei sefydlu yn y Gwasanaethau Oedolion, sef tîm gwaith cymdeithasol cymunedol oedolion sydd wedi'i alinio'n ddaearyddol ac sy'n gwasanaethu ardal Gogledd Abertawe.
Gan weithio o fewn lleoliad amlddisgyblaethol a strwythur goruchwylio cefnogol, mae'r rolau'n cynnwys cynnal asesiadau ac adolygiadau cymhwysedd gofal cymdeithasol statudol a glynu wrth egwyddorion diogelu, wrth gydweithio'n greadigol â dinasyddion i hyrwyddo eu hannibyniaeth a'u lles.
Rhaid i ymgeiswyr fod yn gymwys i weithio fel Ymarferydd Gwasanaethau Cymdeithasol yng Nghymru, bod â phrofiad o waith cyflogedig neu wirfoddol mewn lleoliad gofal cymdeithasol a gwybodaeth am Ddeddf Gwasanaethau Cymdeithasol a Llesiant (Cymru) 2014. Mae safonau uchel o sgiliau cyfathrebu ac ysgrifennu adroddiadau yn hanfodol.
Mae amrywiaeth o opsiynau contract ar gael, gan gynnwys swyddi amser llawn dros dro, rhan-amser dros dro, a rhan-amser parhaol. Mae swyddi dros dro yn para 12 mis.
Y cyflog ar gyfer y swydd yw Gradd 7 £33,119 - £36,581 y flwyddyn neu pro rata ar gyfer swydd ran-amser.

- Conduct appropriate and proportionate adult social care assessments.
- Produce clear, concise case reports and documentation.
- Identify, assess and manage risk in line with safeguarding policies.
- Collaborate with multidisciplinary partners to coordinate care.
- Review and update care plans to reflect changing needs.
- Provide advice and support to service users and their families.
- Participate in team meetings, case conferences and supervision.
- Contribute to service development and continuous improvement initiatives.
- Maintain up‑to‑date knowledge of relevant legislation and best practice.
- Promote the independence and wellbeing of adults in the community.
- Paid or voluntary experience in a social care setting.
- Qualified Social Services Practitioner status in Wales.
- Knowledge of the Social Services & Wellbeing (Wales) Act 2014 and related legislation.
- Excellent verbal and written communication skills.
- Strong report‑writing ability.
- Ability to work both independently and as part of a multidisciplinary team.
- Understanding of adult safeguarding principles.
- Creative approach to engaging and supporting citizens.
- Commitment to professional development and reflective practice.
- Eligibility to work in Wales.
