This position would be based in our Merthyr office, with the role requiring support in the community across our RCT and Caerphilly services in Hirwaun, Penywaun, Rhigos, Aberdare, Cwmbach, Mountain Ash, Penrhiwceiber, Ynysybwl, Abercynon, Cilfyndd, Glyncoch, Pontypridd Town, Rhondda, Trallwn, Graig, Treforest, Rhydfelin, Hawthorn, Tonteg, Church Village, Llantwit Fardre, Taffs Well, Trecenydd, Penyrheol, Bedwas, Aber, Energlyn, Aberbeeg, Oakdale, Rhymney, Bargoed, Markham, New Tredegar and Fochriw, so local knowledge of the areas would be of benefit.
This role will involve working with the Care Coordinators in your assigned area to support care packages ensuring that the needs of service user are at the centre of everything we do. As a Field Care Supervisor, you will supervise and support the development and performance of our Care Workers out in the field to ensure they are delivering the best quality care for our service users, to the highest standards.
Your main duties will include:
Maintaining contact with existing service users, monitoring and reporting on a daily/weekly basis
Providing support to Care Coordinators regarding continuity of care, taking into account locations, skills required and service user preferences
Taking responsibility for new referrals from private clients and social services teams, ensuring full information is received and appropriate care planning is undertaken
Ensuring that service user risk assessments are carried out in a timely manner, thereby eliminating or mitigating against risks to both Service User and Care Worker
Providing practical support of care for Service Users with more complex care needs, including creation and updating of care plans
Providing both on and off the job support and supervision for Care Workers, providing guidance, training, shadowing, continuous spot checks, and performance appraisals
Taking part in the out of hours and on-call rota

NVQ Level 3 or equivalent in Health & Social Care
Background in Care work and a good understanding of the social care sector.
Computer literacy, Excel, Word, PowerPoint, e-mail
Excellent understanding of English language both written and spoken
Experience of managing/supervising people
A full UK driving license
