About the role: We are currently seeking a dedicated and proactive individual to join our team as a Community Social Worker within the Complex Disability Team. This pan-disability team provides support to individuals who have complex needs, including learning and physical disabilities, acquired brain injuries, and those who are neurodivergent. Our primary aim is to support individuals in achieving their personal outcomes, whilst carefully considering both their health and social care requirements.

As a Community Social Worker within the Complex Disability Team, you will deliver community‑based social work support to adults with disabilities, ensuring their needs, outcomes and aspirations are at the centre of all intervention. You will work collaboratively with individuals, families, carers, and partner agencies to promote independence, wellbeing, and progression in line with the Social Services and Wellbeing (Wales) Act 2014.
Key Responsibilities: - Carry out strengths‑based assessments, identifying outcomes with individuals and their support networks.
- Develop and review outcome‑focused care and support plans, ensuring access to community resources and services.
- Manage a caseload effectively, including complex cases where required.
- Work jointly with health, voluntary sector and multi‑agency partners, contributing to meetings and case conferences.
- Ensure timely and accurate record‑keeping compliance with legislation, policy and safeguarding procedures.
- Promote equality, uphold confidentiality, and maintain a safe working environment.
- (For experienced workers) Provide supervision, mentoring and professional guidance to students and less experienced staff.
About you: - A recognised Social Work qualification (BA Social Work, DipSW, or CQSW) and a passion for applying it where it matters most.
- Experience supporting individuals and families, whether gained in Adult Services, Children's Services or both - with the ability to transfer your skills confidently into community disability practice.
- Strong assessment and analytical skills, enabling you to identify what really matters to individuals and build effective, outcome‑focused plans.
- Confidence in managing risk, making sound professional judgments and supporting people in a way that promotes independence and safety.
- Excellent communication skills - written, verbal and relational - allowing you to build trust with individuals, families, and multi‑agency partners.
- A collaborative mindset, thriving in team environments and valuing shared problem‑solving.
- The ability to prioritise and manage your own workload, even when dealing with complex or fast‑paced situations.
- A commitment to continuous professional development, staying curious, reflective and eager to grow in your practice.
