[Internal] Reablement Practitioner x5 - Integrated Reablement Service

Your role as a Reablement Practitioner offers a unique opportunity to work as part of a multi-professional team. To use your knowledge, skills and attributes to assess, enable, support, supervise and assist individuals following illness and\/or hospital stay to maximise and maintain their independence in a way that is focused on what matters to them and their personal outcomes.
This service operates between 07:00am-23:00 pm, 7 days a week including bank holidays and will involve weekend working on a rota basis in line with Torfaen Councils Domiciliary Care Service.

Welsh Language Skills are desirable for this post but not essential.
