We are looking to recruit a Social Work Assistants to support the Home First and Newport Hospital Discharge Service.
Home First is an initiative established across the 5 Local Authorities in Gwent and Aneurin Bevan University Health Board to support people to leave hospital at the earliest opportunity. The team consists of qualified and non-qualified staff who work across three acute sites within the Gwent Region. The team manage and oversee the requirements to support discharge on behalf of the 5 Local Authorities and work as part of a multi-disciplinary team approach in order to achieve positive outcomes for individuals and carers.
The service is based across three hospital sites, The Grange University Hospital, Nevill Hall Hospital, Abergavenny and Royal Gwent Hospital, Newport and is predominantly targeted at discharges from the preadmission wards.
The Home First Service has established itself as instrumental in supporting the discharge pathways and is a really exciting opportunity for staff to influence discharge and admission practices across our acute sites and improve the hospital experience for our citizens.
The Service covers seven days a week and whilst the current roles are Monday to Friday, there is the opportunity for weekend work.
The Social Work Assistant role will be key in taking referrals and coordinating information and supporting colleagues with the discharge of individuals and patient flow.
This post is exempt from Rehabilitation of Offenders Act (1974) and is subject to a satisfactory Disclosure and Barring Service (DBS) check.
