Social Work Assistant Job information Post No. POSN004452 - Social Work Assistant
Newport City Council is seeking dedicated and compassionate Social Work Assistants to join our dynamic Adult Social Services Directorate. These teams support adults aged 18 and over, from the front door, in the community and to support hospital discharge, working collaboratively to promote well-being and independence in line with the Social Services and Well-being (Wales) Act 2014.
The teams across the directorate are made up of our Information, Advice and Assistance team, located in the Civic Centre, acting as a preventative service, to avoid the need for statutory intervention, signposting to other support services, as well as third sector agencies. The Neighbourhood Community Network teams, who work closely with those in the community who are supported by statutory services. As well as the Hosptial team, who work with Newport residents across ABUHB health board, who have come into hospital in crisis, and require ongoing assessment and discharge planning, to enable them to return home at the earliest opportunity.
Key Responsibilities:
- Manage the daily duty function within the team, including answering the phone, managing the email duty inbox, complete proportionate assessments tailored to the persons needs and make changes or create care plans. - Respond to referrals and complete lateral enquiries as required to inform assessment and decision making. - Deliver Information, Advice, and Assistance (IAA) to citizens, where prevention is high on the agenda, with consideration of exploring a persons own hierarchy of support in the first instance. - Work in partnership with health services and community agencies to ensure a multi-agency approach is applied to assessment and outcomes, enabling a more holistic view of the individuals needs. - Contribute to outcome-focused planning that reflects what matters most to individuals, and delivering a person centred approach.

- A passion for supporting adults in need of care and support, and promoting their well-being and independence. - Strong communication, both verbally and written, as well as interpersonal skills that enhance quality of completed work. - Ability to work collaboratively in a fast-paced, team-oriented environment to ensure outcomes are met in the least restrictive manner. - Creativity and openness to new ideas in social work practice, in line with current legislation, policy and theory, in teams that have experienced staff to aide learning and development. - Previous experience in a social care or support role is desirable, but not essential, however, you will demonstrate a keen drive to learn.
